Body Mass Index was created by a Belgian mathematician in 1832. It divides your weight by height squared. That is the entire formula. It cannot tell whether your 80 kg frame is 15% body fat with dense muscle or 35% body fat with visceral deposits around your organs.
In India, this matters enormously. The TOFI phenotype (Thin Outside, Fat Inside) is prevalent in the Indian population — individuals who appear lean by BMI standards but carry dangerous levels of visceral fat. A 2019 study in The Lancet Diabetes & Endocrinology found that up to 30% of normal-weight Indians have metabolic obesity.
Dual-Energy X-ray Absorptiometry uses two X-ray beams at different energy levels to differentiate between bone mineral, lean tissue, and fat tissue. The result is a regional body composition map showing:
Total body fat percentage — not just weight, but where the fat is distributed.
Visceral fat area — the metabolically dangerous fat surrounding your organs.
Lean muscle mass by limb — critical for detecting sarcopenia (age-related muscle loss).
Bone mineral density — osteoporosis risk assessment.
Peter Attia calls body composition "the most underappreciated metric in health." Visceral fat is the strongest predictor of metabolic syndrome, type 2 diabetes, and cardiovascular disease — and it is invisible to BMI, waist circumference, and your bathroom scale.
