Total cholesterol
All the cholesterol in your blood, the good and the bad, added up into one number.
Also printed asTC · Cholesterol Total · Serum Cholesterol · Cholesterol
What it measures
All the cholesterol in your blood as one number: the LDL that can settle in artery walls, the HDL that clears it, and a share riding with blood fats.
On its own a blunt summary of heart risk. Two people with the same total can be in very different shape; what matters is the split between LDL, ApoB and HDL.
Why it matters for how long you stay well
Cholesterol earns its reputation through the particles that push into artery walls over decades, so the useful long-run question is which part of this total is high, and for how many years, not the total itself. A modest total with a poor split ages arteries faster than a higher total with a good one.
How it shows up in daily life
You cannot feel it. It is the headline figure your lab prints first, but it hides which part is rising: the same total can be mostly good or mostly bad.
High and low readings
When it reads high
More cholesterol is circulating than the heart likes over decades. Usually it is the LDL share that has risen, so ask for the breakdown before reading anything into the total.
When it reads low
Low total cholesterol is generally fine and often goes with an active life or a lean diet; only a very low value alongside weight loss or illness needs a look.
What moves it
Daily life
- saturated fat from ghee, coconut, red meat and full-fat dairy
- fibre from dal, oats, vegetables and fruit
- weight around your middle
- an underactive thyroid, which lifts it
- aerobic exercise
Medicines known to shift it
- statins (lower it)
- thyroid hormone replacement (lowers it when the thyroid was slow)
- oral contraceptives (can raise it)
- steroids (can raise it)
Listed so a result can be read in context. It is not advice to start, stop or change any medicine; that decision belongs with your physician.
Read it with
Markers that move this one, or that this one moves. One number read alone can mislead.
- TSH, the thyroid signalKnown mechanism
TSH pushes it up
The same slowdown does not stop at LDL. Everything the liver is meant to collect backs up behind it, so the total drifts up as well.
- Non-HDL cholesterolKnown mechanism
As this rises, Non-HDL cholesterol is calculated from it
Non-HDL cholesterol is your total with the protective fraction taken out. It is subtraction on a result you already have, not a separate test.
Common questions
What does Total Cholesterol measure?
All the cholesterol in your blood as one number: the LDL that can settle in artery walls, the HDL that clears it, and a share riding with blood fats. On its own a blunt summary of heart risk. Two people with the same total can be in very different shape; what matters is the split between LDL, ApoB and HDL.
What does a high Total cholesterol result usually mean?
More cholesterol is circulating than the heart likes over decades. Usually it is the LDL share that has risen, so ask for the breakdown before reading anything into the total.
What does a low Total cholesterol result usually mean?
Low total cholesterol is generally fine and often goes with an active life or a lean diet; only a very low value alongside weight loss or illness needs a look.
What is the usual reference range for Total cholesterol?
A broad adult reference band is 120–200 mg/dL. Labs differ, so read your result against the band printed on your own report.
Educational only, not a diagnosis. A single number means little without your history and the rest of your panel, so discuss your own results with your physician.
Typical adult reference band
- Adults
- 120–200 mg/dL
Labs differ. The band printed on your own report is the one that applies to your result.
Reported in mg/dL, mmol/L
Ageing relevance
3/5
A moderate ageing association; used in clinical longevity panels.
Worth knowing
Your liver makes most of the cholesterol in your blood; only a small share comes from what you eat.
