LDL, the carrying cholesterol LDL Cholesterol
The cholesterol that gets left in the wall of an artery.
Also printed asLDL · LDL-C · Low Density Lipoprotein · LDL Cholesterol (Direct) · LDL Cholesterol (Calculated)
What it measures
The cholesterol carried on particles that can settle into artery walls over years.
The main driver of heart attack and stroke risk; lifetime exposure matters more than any single reading.
Why it matters for how long you stay well
How much of this your arteries see, multiplied by how many years they see it, is the single best-understood driver of heart attack and stroke, and it is one of the most changeable numbers on this page.
How it shows up in daily life
You feel nothing while it builds, which is the whole problem. The damage is silent for decades and shows up all at once.
High and low readings
When it reads high
Higher LDL over decades raises heart risk; the trend matters more than one reading.
When it reads low
Low LDL is protective for the heart.
What moves it
Daily life
- saturated fat from ghee, coconut oil, red meat and full-fat dairy
- soluble fibre from oats, dal and vegetables
- an underactive thyroid, which lifts it
- weight around your middle
- smoking, which damages the wall this settles into
Medicines known to shift it
- statins (lower it)
- thyroid hormone replacement (lowers it when the thyroid was slow)
Listed so a result can be read in context. It is not advice to start, stop or change any medicine; that decision belongs with your physician.
Read it with
Markers that move this one, or that this one moves. One number read alone can mislead.
- TSH, the thyroid signalKnown mechanism
TSH pushes it up
Your thyroid sets the speed at which your liver pulls LDL out of the blood. When the gland idles, the clearing slows and LDL piles up.
- Free T4, the storage hormoneKnown mechanism
Free T4 pulls it down
Free T4 is the hormone that actually gives the order. More of it speeds the liver's clearing of LDL, and less of it lets LDL linger in the blood.
- ApoB, the particle countKnown mechanism
ApoB moves with it
ApoB counts the particles while LDL weighs the cholesterol inside them. Two people with the same LDL can carry very different particle counts, and it is the count that tracks risk.
- Triglycerides, blood fatsKnown mechanism
Triglycerides can make it misleading
When triglycerides run high, an LDL figure that was calculated rather than measured stops being reliable. That is when non-HDL or apoB becomes the number to trust.
- Lp(a), the inherited particleKnown mechanism
Lp(a) pushes it up
Lp(a) carries its own cholesterol, and that cholesterol gets counted inside your LDL result. So a high Lp(a) inflates the LDL number without extra LDL particles being there.
- LDL to HDL ratioKnown mechanism
As this rises, LDL to HDL ratio rises with it
This ratio is your LDL divided by your HDL. The two cholesterol numbers already carry the information, so read them rather than the ratio.
Common questions
What does LDL Cholesterol measure?
The cholesterol carried on particles that can settle into artery walls over years. The main driver of heart attack and stroke risk; lifetime exposure matters more than any single reading.
What does a high LDL result usually mean?
Higher LDL over decades raises heart risk; the trend matters more than one reading.
What does a low LDL result usually mean?
Low LDL is protective for the heart.
What is the usual reference range for LDL?
A broad adult reference band is 40–100 mg/dL. Labs differ, so read your result against the band printed on your own report.
Educational only, not a diagnosis. A single number means little without your history and the rest of your panel, so discuss your own results with your physician.
Typical adult reference band
- Adults
- 40–100 mg/dL
Labs differ. The band printed on your own report is the one that applies to your result.
Reported in mg/dL, mmol/L
Ageing relevance
3/5
A moderate ageing association; used in clinical longevity panels.
Worth knowing
Cholesterol cannot dissolve in blood, so it has to travel wrapped inside these particles, like cargo inside a boat.
